The Single Pathway is a way of ensuring that children with complex and significant needs can access appropriate services in order to receive suitable support. A team of experienced professionals from Wandsworth education and Early Years and Intervention Support Services (EYISS), therapies, community paediatric service look at the referrals every two weeks and recommend the right services.
1. Children aged 0-5 living in Wandsworth, registered with Wandsworth GP and who:
- Have complex needs (Level 3 or 4 spectrum of needs) that require them being assessed by a team of professionals from different specialist services (Multi Disciplinary Assessment) or complex needs Health services e.g. speech and language therapy, occupational therapy
2. Children aged 0-5 living in Wandsworth, registered with non-Wandsworth GP and who:
- Have complex needs (Level 3 or 4 spectrum of needs) that require them being assessed by a team of professionals from different specialist services.
Some Wandsworth Health services will not be able to engage, unless the child is registered with Wandsworth GP.
3. Children and young people aged 0-18 living in Wandsworth, registered with Wandsworth GP and who:
- Need help in their development, because they are not making progress and are considered to have significant or complex needs
- Are already receiving help from specialist services but now may need more or different support.
A parent/carer and a professional who already works with the child or knows about child’s needs should in first instance discuss making a referral together. This should be recorded using an Early Help Assessment, which can be done electronically or on paper. With parent permission information contained in the Assessment will be stored and shared with relevant services. The Assessment should give enough information about child’s needs to enable the Single Pathway Planning Team to make a decision about which services are most suitable to support your child. Any relevant documents like medical reports, teacher’s observation notes should be enclosed with the Assessment. If the child has already been assessed by a social worker then this assessment can be used instead providing that information about child’s development is included. The Assessment or social work assessment should be sent to the Single Pathway, Early Years and Intervention Support Service (EYISS), 1 Siward Road London SW17 0LA. If your child attends a nursery or school, parent/carer should contact the Special Educational Needs co-ordinator (SENCO) or Inclusion Manager or any involved professional in child’s setting, to discuss their concerns, first.
If your child does not attend a nursery or school, parent/carer could ask child’s health visitor or GP or any other professional who knows the child well, to make a referral.
