Preparing for Adulthood is the part of the Special Educational Needs and Disabilities (SEND) reforms which aims to support young people to move into adulthood with fulfilling lives. It is sometimes called "Transition" and includes young people from 14 up to 25 years old.

Young people have told us that the following four outcomes are important to them as they approach and go into adulthood:
- Paid employment (including self employment)
- Good Health
- Independent Living (choice and control over your life and support and good housing options)
- Community Inclusion (going out with friends, relationships)
Young people have told us that they want more freedom, with choices about their future life.
Planning for this takes time and needs to start early on. Preparing for adulthood should be talked about and thought about at school, at home and with all the professionals who work with young people. This is especially important as a young person enters school Year 9 and moves into and through their education after they are 16.
This can be complex, as several professionals may be working to support a young person. Who these professionals are, will change as a person becomes older. As young people turn 18, the law changes and, with that, comes a change in the approach of professionals. Finding your way through the information and the changes can be daunting. It is important that the services are available to help you and the information on this website is designed to assist you through the process.
Who can help?
The Adult Care Information Service (ACIS) has information about services for people with disabilities aged 18 and over.
The 0 to 25 Disabilities Team is a social work team which works with children and young people with severe and enduring disabilities from 0-25 years old and their parents/carers.
