0 to 25 Disability Team, Social Care Referral Eligibility Criteria

The 0 to 25 Disability Social Work Team is a social work team working within Children's Services. It can accept referrals for social care for a disabled child or young person aged 0-25 years old.
Due to the way the law is arranged there are two sets of legislation covering eligibility for childrens (0-17) and adults (18 to 25th birthday) referrals pathways.
Referral for children 0-17 years old:
If you want to do this, the child or young person needs to meet these referral eligibility criteria:
- complex health and developmental needs, or
- a diagnosis of Autistic Spectrum Disorder (ASD) with an associated severe learning disability, or
- sensory disabilities, or
- a severe physical disability, or
- a life limiting condition.

Referrals for adults up to 25 years old:
Referrals can be taken as early as 14 however services (if considered eligible) would not start until the young person’s 18th birthday
Condition 1
The adult’s needs for care and support arise from or are related to a physical or mental impairment or illness and are not caused by other circumstantial factors.
This includes if the adult has a condition as a result of physical, mental, sensory, learning or cognitive disabilities or illnesses, substance misuse or brain injury.
Condition 2
As a result of the adult’s needs, the adult is unable to achieve two or more of the outcomes specified in the Care Act 2014 regulations and outlined below;
- Managing and maintaining nutrition
- Maintaining personal hygiene
- Managing toilet needs
- Being appropriately clothed
- Being able to make use of the adult's home safely
- Maintaining a habitable home environment
- Developing and maintaining family or other personal relationship
- Accessing and engaging in work, training, education or volunteering
- Making use of necessary facilities or services in the local community, including public transport, and recreational facilities or services
- Carrying out any caring responsibilities the adult has for a child
Condition 3
As a consequence of being unable to achieve these outcomes, there is, or there is likely to be, a significant impact on the adult’s wellbeing, determining whether:
- the adult’s needs impact on at least one of the areas of wellbeing in a significant way or
- the cumulative effect of the impact on a number of the areas of wellbeing means that they have a significant impact on the adult’s overall wellbeing.

What is Wandsworth’s Local Offer ?
To use this service you can refer your child/young person 0 to 17, yourself, through the Multi-Agency Safeguarding Hub (MASH), or they could to be:
- Referred by the Multi-Agency Safeguarding Hub (MASH)
- Referred by the Special Educational Needs Co-Ordinator (SENCO) at your child's Special School.
