Learning Disability Nursing Team
The Learning Disability Liaison Nursing Team is a service provided to support adults (18 years +) with a formal diagnosis of a learning disability to access St Georges Hospital.
The objectives of the service are to enable patients with a learning disability to access high quality care and treatment, to work with members of the multi-disciplinary team to ensure patients remains safe from the point of admission to discharge, to facilitate discussion around best interest decision making under the MCA and to coordinate reasonable adjustments under the Equality Act.
The service provides support to patients accessing various health services on site at St Georges Hospital including inpatient admissions, outpatients appointments and pre-operative assessments.
We accept referrals from all sources, including self-referrals from patients, relatives, carers and members of the multi-disciplinary team.
We do not have a referral form, we accept referrals over the phone or by email.

Referrals to this service can be made by telephone or in writing. Self referrals can be made and requests for support are also accepted from families/carers and professionals or agencies who have an interest in the patient's wellbeing.
