Expert Patients Programme

This is a free course for adults who live with a long-term health condition. It is a self management course which gives you the tools to enable you to take control of your condition. It is run by the Wandsworth Self-Management Service .
The course is run by tutors, who live with long term health conditions. They give practical and realistic advice based on their own life experiences. The course will give you the opportunity to put into practice and develop the self management skills you learn with the support of others who are facing similar challenges.
Long-term health conditions include:
- heart disease;
- diabetes;
- asthma;
- arthritis;
- HIV;
- liver disease;
- back pain;
- depression;
- epilepsy;
- psoriasis;
- incontinence;
- bronchitis; and
- stroke.
Adults caring for someone living with a long-term health condition or disability can take part in Looking After Me, a free course, also run by Wandsworth Self-Management Service.

The programme runs for 2.5 hours
once a week for 6 weeks
with 12-16 people in a group and is led by trained tutors who live with a long-term health condition themselves.
